Steps for enrolling in VA Health Care:
1. PREPARATION: Collect the following information if available:
- discharge papers (i.e. DD214)
- most recent tax return
- SSN for you and dependents
- current insurance program information
- your VA Disability Rating Decision (if applicable)
2. APPLY:

- via online @ www.va.gov
- in person @ One Veterans Drive, Minneapolis, MN 55417
3. REVIEW AND DECISION: Call (877) 222-8387 (press 2) if you haven’t heard back from VA in more than a week from applying.
*NOTE: An accredited representative, like a Veteran Service Officer (VSO), can help with your application and assist you with filing a claim for VA disability.
